Clinical Experience on 100 Cases of Living-Donor Renal Transplantation







Abstract
The one hundred cases of living-donor renal transplantation were performed at Yeungnam University Hospital from January 1984 to July 1993 and were analyzed as follows;
1) Among 100 renal transplantations, 37 recipients received kidney from living related donor(LRD), 63 from living unrelated donor(LURD) and no one from cadever donor.
2) The mean age of recipients was 35 years old with range of 4-58 years.old. The male to female ratio was 2.12:1.
3) The causes of CRF were mostly unknown in 62 cases, chronic glumerulonephritis in 15, hypertension in 10, nephrotic syndrome in 5, acute progressive glumerulonephritis in 3, polycystic kidney disease in 3, vesicoureteral reflux and DM in 1 case.
4) Acute rejections were noted in 6 patients and all of the acute rejection wereoccured in living unrelated transplantation. 4 Cases were recovered after steroid pulse therapy in 3 cases and OKT3 rescue therapy in 1 case. Recovery rate was 66.6%.
5) Chronic rejections were noted in 9 patients. The incidence was more frequent in living unrelated transplantation (11%) than in living related transplantation (5%).
6) Graft losses after transplantation were ocurred in 17 cases. Among 17 cases, 2 cases due to irreversible acute rejection, 9 cases due to chronic rejection and 6 cases due to patients death were noted. Suspected death causes were pneumonia in 3(1:CMV, 2:unknown), cerebrovascular accident in 1, hepatic failure in 1, varix bleeding due to liver cirrhosis in 1 case.
7) The complications associated with immunosupression were infection in 12, gum hyperplasia in.3, erythrocytosis in 3, alopecia in 3, hirsutism in 3, CyA nephrotoxicity in 2, DM in 2 and avascular necrosis of femoral head in 1 case.
8) The 3 years patient survival rate was 97% in living related transplantation and 92% in living unrelated transplantation. The 3 years graft survival rate was 94% in living related transplantation and 66% in living unrelated transplantation. This difference was statistically significant(p<0.05).
The graft survival rates according to HLA histocornpatibility and immunosupressive agents in living unrelated transplantation were similar.
The 3 years graft survival rate in living related transplantation was 62.5% in PDS/AZA use group and 100% in cyclosporin A(CyA) use gruop. This result was statistically significant (p <0.05 ).
9) Donor specific transfusion(DST) was performed in 5 patients.
The 3 years graft survival rate in DST group was higher than non DST group without statistic significance.
